According to the Michigan Licensing and Regulatory Affairs (LARA) office the license for Planned Parenthood's operations ended July 31st, 2012.
One concerned Michigan resident, Lynn Mills, has stated that she had written LARA with no response. She said she contacted

However he has yet to contact her back.
Another concerned citizen Abigail Nobel said "I know someone else who was turned away by LARA when they wanted to file a complaint."
Michigan State Senator Patrick Colbeck became of aware of the possible breach and his office will start an investigation on 8/6/15.
If this location is still open as of August 22nd a protest has been planned for this day at this Planned Parenthood location.

I just got back from the protest outside this PP clinic. I spoke with some of the people who have been trying to get information on it, and they say that they are being stymied by the regulatory agency and that it is unclear how it is possible for the clinic to be open legally. Apparently the clinic is in some sense "operating under" the physicians' licenses of the doctors who work there. (And that is another whole story, because one of the doctors there was notorious in Illinois and then came to "bless" Kalamazoo with her presence.) But the point is that if the facility itself is supposed to be licensed, physician licenses should not be sufficient. So what's the deal?
